HealthCare Support is actively seeking a Hybrid Bilingual Licensed Care Coordinator - LVN / LSW to fill an opening with a fortune 25 Healthcare company.Daily Responsibilities for Hybrid Bilingual Licensed Care Coordinator - LVN / LSW
- Complete Face to Face, Zoom, or Telehealth assessments, up to 4 assessment per day x3 days per week
- Complete required forms for all assessments completed within required time frames, Documentation days x2
- Work assigned tasks/assignments to address member needs/requests
- Shift: m/f 8-5 CST
- Location: Hybrid. visits will be primarily in Hidalgo county, but could travel to Maverick, Webb, Zapata, Starr, Willacy, Cameron, Jim Hogg, Duval or McMullen counties
- Speak/Read/Write English/Spanish
- Clear and active LVN or LSW license
- 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions
- Field/Home Health Experience- Must be comfortable going into members' homes/field work.
